How to make a referral
Referrals can be made by calling our friendly reception staff on 03 9458 4845 or by completing the online referral form here.
Communication
The team may contact you to discuss your referral and will communicate with you on a regular basis in relation to your patient’s progress. If you have any questions or wish to discuss your patient’s progress or care, please do not hesitate to contact us on 03 9458 4845.
